WB Swasthya Sathi Form PDF for enrollment under স্বাস্থ্য সাথী কার্ড নাম লিস্ট 2024 for all. West Bengal government has decided to extend Swasthya Sathi benefit to all residents of the state, except those who are already covered under any Govt, Sponsored Health Insurance, or Assurance Scheme. New member addition in Swasthya Sathi Card (FORM A)

You need to submit form A in case you want to add the name of a new family member to your Swasthya Sathi card. It also needs to be submitted in case the name of one of the members of your family or beneficiaries is missing from your Swasthya Sathi card.

To add a new members name to your Swasthya Sathi card, follow;

1. Download Swasthya Sathi Form A from the above-mentioned link.
2. Fill up all the necessary details like Beneficiary Name, Address, URN number, etc.
3. Add the names and details of the members that need to be added.
4. Submit the form along with the required documents at your nearest Duare Sarkar camp when it resumes.

Swasthya Sathi card application form (FORM B)

You need to fill up and submit form B to apply for a Swasthya Sathi card in West Bengal.

To apply for a new Swasthya Sathi card, follow;

• Fill up all the necessary details like, Address, caste, and other details properly.
• Next, fill in the name and details of all the members/ beneficiaries in your family.

Swasthya Sathi card name correction (FORM C)

In case you have received your Swasthya Sathi card or checked your name in the name list you might find a wrong or misspelled name. The Govt of West Bengal has released form C to help you correct your or any of your family members’ names on this card.

To correct the names of you/members in the Swasthya Sathi card, follow;

1. Download Swasthya Sathi Form C from the link above.
2. Fill up all the necessary details like Address, Beneficiary name, URN number, etc.
3. Enter the names that need to be corrected.
4. Submit the form along with the required documents at your nearest Duare Sarkar camp when it resumes.

Deletion of member in Swasthya Sathi Card (FORM D)

Sometimes there may be the wrong member addition in your Swasthya Sathi card. Or there may be some other reasons for which you need to delete a member/ beneficiaries from your card.

To remove or delete a member’s name from your Swasthya Sathi card, follow;

• First, download Swasthya Sathi Form D.
• Then fill up all the necessary details like Address, Beneficiary name, URN number, etc.
• Enter the names of the members that need to be deleted.
