  6. In case you choose Option 2, then you have to enter a name, father’s name, age, date of birth, gender occupation, address, mobile number, PAN number, username, email, etc.
  7. Upload your photograph and other documents here.

  2. On the web homepage, press the ‘Verify Your Authentication Certificate’ option.
  3. Now you have to choose the name of the department and sub-department from the various options.
  4. Then choose the name of the ‘Service’ for which you applied.
  5. Enter the Application ID and click on the ‘Go’ box.
  6. A new page will be open up, where you will have to enter an 18 Digit Barcode Value to verify your certificate.

I applied for death certificate of Ajit Mhatre using option in Rural Development and Panchayat Raj Dept on 01/04/2019 application id – 1951741702480600006438 and expected date was given as 06/04/2019. I paid 20 + charges = 23.60. Till date i have not received the certificate. I even made an appeal but no actin has been taken.

    I have again applied for the same on 19/08/2021 application id – 2151741702480600073310 and expected date was 24/08/21. i have paid 20 + charges for the same.

    I fail to understand If services are not attended to why is this portal given. If Services are not provided then the amount should be refunded and the portal Closed.
